Poor Vitamin D Status Is Associated With Chronic Pain In The Elderly

In a recent population based study of men and women over 65 years of age, those experiencing chronic moderate to extreme pain were twice as likely to be vitamin D deficient than those not reporting pain. 

Inadequate vitamin D levels are common in older adults and may result in osteoporosis, osteoma-lacia and a wide range of other non-communicable diseases that have potential effects on health outcomes. Moderate to extreme pain is also more likely to present in older adults. 

A recent study published in the British Journal of Nutrition has shown that older men and women experiencing moderate to extreme chronic pain are more likely than others to have poor vitamin D status. 

Researchers sought to examine the connection between chronic pain and serum vitamin D levels among a population of men and women aged 65 years or older. The study included 2,070 adults who took part in the 2005 annual Health Survey for England, which assessed health and health-related behaviors in children and adults. 

The results of this study show that symptoms of chronic pain were associated with poor vitamin D status, independent of other variables. Of the 53% of participants that reported experiencing moderate or extreme pain or discomfort, 80% had chronic illnesses and 60% had been diagnosed with musculoskeletal conditions. 

The odds of experiencing moderate to severe pain increased with decreasing levels of serum vitamin D.  In those reporting pain, the odds of being deficient in vitamin D (less than 25 nmol/l, or 10 ng/ml) were double that of subjects who did not report pain.
 
These results support previous research showing an association between increased pain and poor vitamin D status. The researchers suggest that the active form of the vitamin may be associated with a reduction in inflammation. The results of this study also support the supposition that it is important to screen older patients reporting pain for vitamin D status so that appropriate interventions can be provided.

I attended a talk by a specialist doctor in Auckland this week.

One of the many interesting things that I learned from him was that having a too low cholesterol level  (i.e. < 4.5/171) increases the risk of Alzheimer’s Disease.

Iowa Women's Health Study flawed

Recently there have been headlines in the media about the Iowa Women’s Health Study results that proclaims that dietary supplements have no value or are even dangerous.

Self-care for breast cancer prevention

I am suspicious of so-called charities that raise money for breast cancer because it seems to be all about the research for survival rather than teaching women about prevention.

This blog post by Dr Christiane Northrup provides the information we need to have healthy happy breasts!  After all, prevention is easier than survival.

Releasing the Fat - the beginning

I am going to be quite bold and write a diary about my current project which is to release the excess 10kgs of fat around my middle.

I didn’t have any problems with weight as a child/teenager.  I was termed ‘skinny’.  I have always had a healthy diet and exercised.  I lived in the UK for 16 years and did heaps of cycling and walking while I lived there.

My son was born in 1989 and my weight dropped off easily after giving birth.  But the same thing didn’t happen after my daughter was born.  I was kept busy looking after two children, a house and a husband so my needs were at the bottom of the pile.  I still ate healthy food but I just didn’t have the time to exercise.  So the excess weight clung on.

As the children got older I did get out and join Weight Watchers. And yes, I did release weight.  And then I put it back on again.

Some years later I joined SureSlim.  Again, I released weight.  And then I put it back on again. Anyone else familiar with this kind of story?

This time has got be different…

I have consulted with David Holden, Naturopath.  And he has put me on a program that has been a success for his clients.

This is how I looked in June 2011 (while I was fishing in Australia).

Micronutrient supplementation Benefits critically ill adults


Elderly and critically ill people are more likely to have lower energy intakes and lower intakes of important micronutrients.  Ensuring optimal nutrition is very important in promoting the health and well-being of older adults.

A latest study published in the July 2011 issue of Nutrition assessed the effects of micronutrient supplementation on adults recovering from critical illness. Primary outcomes included clinical endpoints (mortality, infectious complications, length of intensive care unit and of hospital stay). Secondary outcomes included descriptions of practice issues, micronutrient status, morbidity, course of the acute-phase response, and oxidative stress.

Thirty-three randomized-controlled trials were included for the primary and secondary objectives.  Fourteen trials (n = 1468) showed a statistically significant decrease in overall mortality. Six RCTs (n = 1194) indicated a statistically significant decrease in 28-d mortality. Micronutrient supplementation was not associated with a decrease in infectious complications, length of intensive care unit, or length of hospital stay. In subgroup analyses, a sensitivity analysis of combined micronutrients indicated a significant decrease in mortality. The secondary outcomes confirmed that timing, duration, and dosing of supplementation appear to be key factors to ensure optimal clinical benefit.

The study authors conclude that there is a potential benefit of micronutrient supplementation in critically ill adults by possibly being associated with a decrease in mortality.

Is your cat really healthy?

When I have warned cat owners about the dangers of a dry food diet, they tell me that their cat has been checked out and is healthy.

But is it really healthy?

Has your vet checked your cats blood sugar levels?

My cat’s last annual check up in October 2010 pronounced him healthy.  But his blood sugar levels were not checked.

Unless this is done and you feed your cat dry food, you don’t really know if your cat is healthy.

Caesar was eating a mixed diet of Purina One, Whiskas and raw pork.

 If I had stopped the dry food he may have died of old age instead of feline diabetes.

If you look around you at your fellow humans, you will be aware that there is a growing problem of obesity that leads to Diabetes in humans – humans are eating diets that contain too much processed carbohydrate, and not getting adequate exercise.  This results in a sick society.  It happens over a period of years.

Which is the same for cats.  They are not meant to eat carbohydrates – Pet food ingredients that contribute to feline diabetes

But don’t take my word for it – do your own research.  Start by searching ‘feline diabetes’ in your favourite search engine.

Can vets help prevent feline diabetes?

As you can see from my  cat Caesar’s Health Record below, I diligently took Caesar for his annual check up.

My suggestion for vets is to ask the cat owner about the cat’s diet when they come in for their annual check.

If the cat is overweight, or  is fed dry food or is over 7 years old, then it’s blood sugar levels should be checked.

And I am sure there is a simple way to do this with a glucose meter by obtaining a blood sample with a lancet via an ear prick or paw prick, rather than having to send samples off to a lab (which you would do if the reading comes back high).

Rather than wait for the symptoms of feline diabetes (The first obvious symptoms are a sudden weight loss (occasionally gain), accompanied by excessive drinking and urination; for example, cats can appear to develop an obsession with water and lurk around faucets or water bowls. Appetite is suddenly either ravenous (up to three-times normal) or absent. In cats the back legs may become weak and the gait may become stilted or wobbly (peripheral neuropathy)) to become apparent.

Once the symptoms of feline diabetes appears, then it may be too late to save your cat’s life (which happened to my cat).

RIP Caesar

Caesar passed away at 12.30pm 19th August 2011.

His health had deteriorated significantly.

Caesar is now at rest.

Caesar did appear to be doing quite well until last night. 

Last night his legs seemed very weak and all he did was lie very close to our fire.

This morning he has had only a nibble of food.

He is not grooming himself and today he is just sitting in front of the heater.

This morning he accidentally urinated in our bed and just sat there in it.

I am aware that he is very likely on his way out.

This has been a trying week.

Over the weekend he was looking very sickly and went off his food, vomited.

But today he is eating again, went outside to toilet himself, and looks better.

The weather is very cold, windy and wet so he will be staying inside mostly for a few days until this winter storm moves on.

Living with feline diabetes - week 3

This has been a great week for Caesar.

His drinking and urinating have returned to normal. 

His coat is looking healthy.

He is not crying like he was in June.

He is enjoying his new food.

He is being his normal pesky self.

The only issue is that he is still eating more than the 150g a day that should be sufficient. 

However, I expect this will take time to normalise.

On Monday we visited Dr Lyn Thomson at ‘Raw Essentials’

It was very refreshing chatting with her – at last, a vet who understands nutrition.

Afterwards I spoke to our own vet and he is happy with what I am doing to help Caesar.

I bought Caesar raw meat from Raw Essentials – rabbit fillets, Turkey & heart mix and Heart mince.

That’s just for the first two weeks.

Then he will be enticed by chicken, meat bones, organ pieces and green tripe (stomach contents).

What had my cat been eating?

Before my cat Caesar had been diagnosed with diabetes, I had trusted pet food manufacturers and had never read a label.

Even if I had read the labels, they would not have read ‘danger’ to me because I trusted the pet food manufacturers.

How misplaced my trust was.

Now I am carefully reading all labels!

Below is a label scan of a dry food that Caesar would eat occasionally.

You will notice that ‘wholegrain cereals’ is the very first ingredient listed. Can you imagine what percentage of the total ingredients that this might be?

And yet,  in the ‘Typical Analysis’ box, carbohydrates are not even listed!

Treating feline diabetes

What choice of treatment do cat owners have when their pet is diagnosed with feline diabetes?

My choice is to change to a 100% raw meat diet – no kibble and no cans with cereals listed as an ingredient.

Since my cat began his new diet this week, I have noticed that his thirst has returned to normal and he is starting to eat normally.

His fur is looking in better condition.

Perhaps eating a 100% raw meat diet is more expensive than processed foods, but it is healthier for our pets.

When I was growing up in Australia, we fed our cat “Timothy Titus” on kangaroo meat. 

He wasn’t allowed inside so he would have supplemented his diet on prey.

He had a long and healthy life.

The other choice of treatment is with insulin.  This can also be expensive. 

And it requires a dedicated owner who can give food and injections at specific times of day.

Living with feline diabetes - week two

It is 2 weeks now since Caesar stopped being given Purina One and Whiskas to eat.

He is drinking less water because he now gets his fluids from the food he is eating.

He is still urinating in his dirt tray a bit more than usual as he mostly stays indoors due to the winter weather, but I no longer have to empty his dirt tray every day.

I had a reply to my letter from Purina One during the week.  You may read it here.

I find it amusing that the letter says ‘In the wild cats would naturally ingest plant material contained in the stomachs of their prey’. 

So in the wild, do prey eat processed foods such as soybean meal, whole grain corn, corn gluten meal, brewers rice?

Raw meat ONLY for cats

This article from Raw Essentials explains why cats should eat only raw meat.

Research Proves It: Cats and Carbohydrates Don’t Mix!

Cats are obligate carnivores, meaning they have nutritional requirements that can only be met with a diet based on animal tissue. The macronutrient profile for cats is high in protein and fat, consistent with a meat and bone based diet.

According to study authors:

The carbohydrate ceiling explains many of the intake patterns seen in both dry and wet diet experiments and suggests that cats may only be able to process ingested carbohydrate up to a certain level.

The feline body is specifically designed for a very low carbohydrate diet.

They have no taste receptors for sweet flavours.

Low rates of glucose uptake in the intestine.

No salivary amylase to break down starches

And a reduced capacity of pancreatic amylase and intestinal disaccharidases

 In other words, cats don’t produce the enzymes required to digest carbohydrates. The only carbohydrates felines eat in the wild are pre-digested and are found in the stomachs of prey animals.

Multivitamin Supplementation Can Have Positive Effects On Body Weight


New research suggests that obese individuals who use a multivitamin/mineral supplement may experience both a decrease in body weight and improved serum lipid profiles.

Obese individuals are more likely to have lower blood concentrations of most vitamins and minerals.  In a new research published in the International Journal of Obesity, scientists evaluated the effects of multivitamin/mineral supplementation on body fat, energy expenditure, and lipid profiles in obese Chinese women.

Ninety-six obese Chinese women between the ages of 18 and 55 participated in a 26-week randomized, double-blind, placebo-controlled intervention study.  Subjects were divided into three groups, receiving either a multivitamin/mineral supplement (MMS), 162mg of calcium, or placebo daily.  Body weight, BMI, waist circumference, fat mass, lean tissue, resting energy expenditure, blood pressure, fasting plasma glucose and serum insulin, total cholesterol, LDL and HDL cholesterol, and triglycerides were measured at the beginning and end of the study period.

After 26 weeks, the multivitamin/mineral group had significantly lower body weight, BMI, fat mass, total and LDL cholesterol, significantly higher resting energy expenditure and HDL cholesterol than individuals in the placebo group.  They were also more likely to have a reduced waist circumference.  The calcium group also had significantly higher HDL cholesterol and lower LDL cholesterol levels compared with the placebo group.

The results suggest that multivitamin/mineral supplementation could reduce body weight and fatness and improve serum lipid profiles in obese women, possibly through increased energy expenditure and fat oxidation.

Vitamin D Deficiency in Pregnant Women


High prevalence rates of suboptimal vitamin D levels have been observed in women who are not considered ‘at risk’.  A recent Australian study suggested that all pregnant women in Australia should take a daily vitamin D supplement.

Vitamin D deficiency is common in pregnant women and in breastfed infants, despite the widespread use of prenatal vitamins. Adverse health outcomes such as preeclampsia, low birthweight, neonatal hypocalcemia, poor postnatal growth, bone fragility, and increased incidence of autoimmune diseases have been linked to low vitamin D levels during pregnancy and infancy.

A cross-sectional study of pregnant women was performed to determine prevalence and predictive factors of suboptimal vitamin D levels in 2 antenatal clinics in Australia – Campbelltown, NSW and Canberra, ACT.

The researchers found behavioural factors, such as sun exposure, were not as predictive of suboptimal vitamin D levels as ethnicity, season and BMI among pregnant woman.  The prevalence of vitamin D deficiency and insufficiency was 35% in Canberra and 26% in Campbelltown.  Among the vitamin D-deficient women, 38% were Caucasian and all had a BMI of more than 30kg/m2. Vitamin D supplementation at 500 IU/day was inadequate to prevent insufficiency.

The study suggested routine supplementation for all pregnant women with a higher dose of vitamin D (1000 IU/day) regardless of their ethnicity or exposure to the sun.  Targeted screening of the women at highest risk of vitamin D deficiency should also be continued and supplemented if necessary.

This is a message we need to give to the young women in our families!  They can help grow healthy adults.

I take 4000iu of Vitamin D daily.  I note that young people that I know do not spend enough time outside in the sun to benefit from getting Vitamin naturally.  Taking supplements is an important option to consider.

A fish-rich diet of the Eskimos' benefits the Heart


Although 70% Alaskan Eskimos are overweight or obese, they did not show the same risk factors for heart disease as the US population.  They also had a lower prevalence of diabetes.  The latest study suggested that an Omega-3 rich diet may offer protection against some of the harmful effects of obesity.

It has been known that Omega-3 fatty acids are associated with favorable, and obesity with unfavorable, concentrations of chronic disease risk biomarkers.
 
In a cross-sectional study, the researchers analyzed data from 330 people living in the Yukon Kuskokwim Delta region of south-west Alaska, who typically consume around 20 times as much omega-3 fats from fish as the average American.  They have similar overweight and obesity levels to those in the US overall but their prevalence of type 2 diabetes is significantly lower, at 3.3% versus 7.7%. 

The researchers examined whether high eicosapentaenoic (EPA) and docosahexaenoic (DHA) acid intakes, measured as percentages of total red blood cell (RBC) fatty acids, modify associations of obesity with chronic disease risk biomarkers.
  
The study found that those with the highest levels of the omega-3 fish oils docosahexaenoic acid and eicosapentaenoic acid had the lowest triglyceride and C-reactive protein levels.  High RBC EPA and DHA were associated with attenuated dyslipidemia and low-grade systemic inflammation among overweight and obese persons. This may help inform recommendations for Omega-3 fatty acid intakes in the reduction of obesity-related disease risk.
